1. Log on to www.gmsupplierdiscount.com

     call 1800-960-3375

2.  Submit your name, SSN, home address and phone number to obtain a GM form        1753. Other forms of ID may be required: ID badge, current pay stub;

3. GM program headquarters validates your employee information and mails a GM form 1753 to your in about 3-5 business days;

4  Determine a participating dealer;

5  Take the GM form 1753 with you to the dealer to confirm eligibility;

6.  Arrange for delivery.

7 The price is GMS price plus 4%. (GMS price is on the sticker)

8. The plan is available to active employees only.
